Overview:
Join our Community Connector Service to support individuals facing complex social circumstances. You’ll help people access activities that promote health and well-being, build support pathways, and maintain accurate case records.
Requirements:
Minimum 2 years’ experience or HNC in Health and Social Care, Community Education, or Social Sciences. Experience working with individuals facing social, emotional, and mental health challenges. Excellent interpersonal, communication, and IT skills. Person-centred approach and commitment to equity.

Overview:
Join our team to support individuals facing complex social circumstances. You’ll strengthen connections between community resources and primary care, enabling people to live well. Working closely with a GP Practice team, you’ll provide person-centred support, build pathways, and maintain accurate case records.
Requirements: Experience working with individuals experiencing social, emotional, and mental health challenges, or complex life circumstances such as homelessness or substance misuse. Strong understanding of challenges faced by people in areas of deprivation. Excellent interpersonal, communication, negotiation, and motivational skills. Solution-focused approach and commitment to non-discriminatory values. IT skills, including word processing, email, and social media. Ability to work independently and as part of a team, with a willingness to undertake training.
